Cheyenne-Laramie County Health Department (CLCHD) Adult Team Nurses work to improve independence and healthy lifestyles of chronically ill, disabled or elderly citizens of Laramie County through education, information and referral to agencies in Laramie County and those funded at CLCHD.

Adult Health Program
Adult Health program provides short term, non-skilled in-home nursing services and personal care for qualified clients. Fees for services are based on a sliding fee scale.

Ryan White Care Grant Services
This program provides services to improve the overall health status of residents living with HIV/AIDS through improved access to health care, coordinating community services and facilitating access to statewide resources. Financial guidelines must be met to qualify.

LT101 Assessment
An LT101 is the functional assessment completed by a Public Health Nurse as part of the qualification process for Medicaid Nursing Home, Long Term Care Home and Community Based Services, Medicaid Waiver and Assisted Living Facility Medicaid Waiver Programs.
