Medical Services for Family Planning Patients:
Counseling:
Education:
Family Planning encourages all new family planning patients to receive instruction on the male and female reproductive anatomy with regards to birth control services. Instruction covers all contraceptive methods, correct use, effectiveness, side effects, possible dangers and advantages and disadvantages of all methods. Instruction also includes abstinence as a viable alternative.
Community Education:
The Family Planning Staff is available for presentations to community groups and schools on a variety of subjects. Feel free to call or stop by our office for more information.
Referrals:
If a patient has a medical concern outside our clinic capability, referrals are made to private physicians for follow-up and possible treatment. Referrals are also made to different agencies for such things as financial assistance, prenatal care, drug and/or alcohol abuse, and counseling.
Cost for Services:
Family Planning provides services to persons of any economic status. As a federally funded agency, patients at 100% of federal poverty level guidelines may receive fully discounted services. Patients are charged on a sliding fee scale based on gross monthly income and size of family. Household income will be verified to establish placement on the sliding fee scale. Suggested forms of verification include most recent tax returns or 3 consecutive paycheck stubs. Donations are greatly appreciated. We accept Medicaid and most private insurances and proof of insurance must be presented upon check in. Express STI/HIV visits also available at an affordable, flat fee rate.
Discrimination Prohibited:
Title VI of the Civil Rights Act of 1964 states: "No person of the United States shall, on the grounds of race, color, national origin, sex, marital status or income, be excluded from participation, be denied the benefit of, be subjected to discrimination under any program or activity receiving Federal financial assistance."
